The bile, which comes from the liver, is also very important for the digestion of fat because it acts like a soap and breaks up the fat into minute droplets so that the pancreatic lipase can digest it. Insulin and Glucose All the body’s cells use glucose (sugar) as an energy source. The level of sugar in the blood is kept constant by insulin, which is made by special cells in the pancreas. If the cells are not working properly and insulin is lacking then diabetes develops. If the Pancreas Stops Working, What Happens? Depending upon how badly the pancreas functions there are two problems. The first is that food is poorly absorbed, which causes weight loss, and there is diarrhea, often rather fatty as the undigested fat causes pale, bulky and smelly motions. Your doctor will focus treatment on your nutritional and metabolic needs and on relieving your pain. Mild pain can be treatment with analgesics. If the cause of acute pancreatitis is gallstones, you may have to have your gallbladder removed to prevent further attacks. If the bile duct is found to be enlarged, you may need an ERCP (endoscopic retrograde cholangiopancreatography) to drain it. An ERCP is a way your doctor can examine your pancreas, pancreatic duct, the common bile duct, and/or sphincter of Oddi. It involves passage of a long narrow tube called an endoscope used to put X-ray contrast dye into the bile and pancreas ducts. In severe cases, surgery will be required to drain the pancreatic duct or to remove part of the pancreas. Your doctor will also likely give you dietary guidelines to follow in order to reduce the amount of fat you eat, since your body has trouble digesting these substances. You may also need to take pancreatic enzyme supplements, which are in the form of a tablet, every time you have a meal. These supplements will help your body absorb food and help you regain some of the lost weight. Can't they transplant the pancreas? There are a very few research hospitals performing pancreas transplants. However, they are limited to kidney and pancreas transplants. It is possible to transplant the islet cells to the liver. The risk of complications is high, and the majority of these procedures have been placed on hold. Are herbs and vitamins important? Many physicians and practitioners are prescribing herbal and vitamin therapy. Many patients report fewer side effects, as well, as improved benefit. Ginger is well known for preventing nausea. There Are Two Stages Of Pancreatitis: Acute pancreatitis: This condition can occur suddenly, soon after the pancreas becomes damaged or irritated by its own enzymes. Acute pancreatitis affects about 80,000 Americans every year. Chronic Pancreatitis: This stage of pancreatitis begins as acute pancreatitis, and becomes chronic when the pancreas becomes scarred. Most cases of acute pancreatitis are mild and involve a short hospital stay to help heal the pancreas. Etiology and Pathogenesis Biliary tract disease and alcoholism account for >= 80% of hospital admissions for acute pancreatitis. The remaining 20% are attributed to drugs (eg, azathioprine, sulfasalazine, furosemide, valproic acid), estrogen use associated with hyperlipidemia, infection (eg, mumps), hypertriglyceridemia, endoscopic retrograde pancreatography, structural abnormalities of the pancreatic duct (eg, stricture, cancer, pancreas divisum), structural abnormalities of the common bile duct and ampullary region (eg, choledochal cyst, sphincter of Oddi stenosis), surgery (particularly of stomach and biliary tract and after coronary artery bypass grafting), vascular disease (especially severe hypotension), blunt and penetrating trauma, hyperparathyroidism and hypercalcemia, renal transplantation, hereditary pancreatitis, or uncertain causes. Symptoms and Signs In pancreatitis, pancreatic enzymes activate complement and the inflammatory cascade, thus producing cytokines. Patients typically present with fever and an elevated WBC count. It may thus be difficult to determine if infection is the cause or has developed during the course of pancreatitis. Most patients suffer severe abdominal pain, which radiates straight through to the back in about 50%; rarely, pain is first felt in the lower abdomen. Pain usually develops suddenly in gallstone pancreatitis versus over a few weeks in alcoholic pancreatitis. Pain is severe, often requiring large doses of parenteral narcotics. The pain is steady and boring and persists without relief for many hours and usually for several days. Sitting up and leaning forward may reduce pain, but coughing, vigorous movement, and deep breathing may accentuate it. Most patients experience nausea and vomiting, at times to the point of dry heaves. The patient appears acutely ill and is sweating. Pulse rate is usually 100 to 140 beats/min. Respirations are shallow and rapid. BP may be transiently high or low, with significant postural hypotension. Temperature may be normal or even subnormal at first but may increase to 37.7 to 38.3° C (100 to 101° F) within a few hours. Sensorium may be blunted to the point of semicoma. Scleral icterus is occasionally present. Examination of the lungs may reveal limited diaphragmatic excursion and evidence of atelectasis. About 20% of patients experience upper abdominal distention caused by gastric distention or a large pancreatic inflammatory mass displacing the stomach anteriorly. Pancreatic duct disruption may cause ascites (pancreatic ascites). Abdominal tenderness always occurs and is often severe in the upper abdomen and less severe in the lower abdomen. Mild-to-moderate muscular rigidity may exist in the upper abdomen but is rare in the lower abdomen. Diagnosis Acute pancreatitis should be considered in the differential diagnosis of every acute abdomen. The differential diagnosis of acute pancreatitis includes a perforated gastric or duodenal ulcer, mesenteric infarction, strangulating intestinal obstruction, ectopic pregnancy, dissecting aneurysm, biliary colic, appendicitis, diverticulitis, inferior wall MI, and hematoma of abdominal muscles or spleen. Laboratory tests cannot confirm a diagnosis of acute pancreatitis but can support the clinical impression. Serum amylase and lipase concentrations increase on the first day of acute pancreatitis and return to normal in 3 to 7 days. Both may remain normal if destruction of acinar tissue during previous episodes precludes release of sufficient amounts of enzymes to raise serum levels. Treatment The need to treat severe acute pancreatitis in an ICU can frequently be determined on hospital day 1 by any of the following danger signals: hypotension, oliguria, hypoxemia, or hemoconcentration (ie, Hct > 50%, indicating severe third space losses). Fluid resuscitation is essential; 6 to 8 L/day of replacement fluid containing appropriate electrolytes and colloid may be required. Severe pain should be treated with meperidine 50 to 100 mg IM q 3 to 4 h prn in patients with normal renal function (morphine causes the sphincter of Oddi to contract and should be avoided). A serum glucose level of 200 to 250 mg/dL (11.1 to 13.9 mmol/L) should not be treated, but higher levels should be treated cautiously with subcutaneous or IV insulin and carefully monitored. Heart failure should be treated by appropriate correction of volume status. Renal failure should be treated by increased fluid replacement if there is prerenal azotemia. Dialysis (usually peritoneal) may also be required. Antibiotic use had been controversial. However, there is now evidence that antibiotic prophylaxis with imipenem can prevent infection of sterile pancreatic necrosis, although mortality is unaltered. Antibiotics should be used to treat specific infections (eg, biliary sepsis, pulmonary infection, UTI). If pancreatic infection is suspected, CT-guided needle aspiration should be performed. Chronic Pancreatitis and Etiology and Pathogenesis In the USA, chronic pancreatitis most commonly results from alcoholism and idiopathic causes. Similar to acute pancreatitis, microlithiasis has been implicated in some cases of chronic pancreatitis. Rare causes are hereditary pancreatitis, hyperparathyroidism, and obstruction of the main pancreatic duct caused by stenosis, stones, or cancer. Rarely, severe acute pancreatitis causes sufficient pancreatic ductal stenosis to impair drainage and result in chronic pancreatitis. Symptoms and Signs 2 Symptoms and signs may be identical to those of acute pancreatitis. Although there is occasionally no pain, severe epigastric pain may last many hours or several days. Possible causes include acute inflammation not recognized by conventional tests, distention of pancreatic ducts caused by strictures or calculi, a pseudocyst, perineural inflammation, or obstruction of either the duodenum or the common bile duct caused by fibrosis of the head of the pancreas. Abdominal pain may subside as acinar cells that secrete pancreatic digestive enzymes are progressively destroyed. Laboratory tests, including amylase and lipase, are frequently normal, probably because of significant loss of pancreatic function. Markers of inflammation (eg, WBC count) are generally minimally elevated as well. Structural abnormalities can be visualized by plain x-ray of the abdomen (showing pancreatic calcification, which indicates intraductal stones), abdominal ultrasound or CT (showing abnormalities in size and consistency of the pancreas, pancreatic pseudocyst, or dilated pancreatic ducts), and ERCP (showing abnormalities of the main pancreatic duct and secondary branches). However, these imaging studies may be normal in the first few years of disease. The most sensitive test of pancreatic exocrine function is the secretin test. It involves positioning a tube in the duodenum and collecting pancreatic secretions stimulated by IV secretin alone or with either cholecystokinin or cerulein. Duodenal contents are collected for volume determination, HCO 3 concentration, and enzyme concentration. A collection that is of normal volume (> 2 mL/kg) and low in HCO 3 (< 80 mEq/L) suggests chronic pancreatitis; low volume (< 2 mL/kg), normal HCO 3 (> 80 mEq/L), and normal enzyme levels suggest pancreatic duct obstruction, perhaps secondary to tumor, and should prompt ERCP. Treatment A relapse of chronic pancreatitis may require treatment similar to that of acute pancreatitis. The patient must eschew alcohol. At times, IV fluids and fasting prove beneficial. Dietary measures of uncertain benefit include small feedings restricted in fat and protein (to reduce secretion of pancreatic enzymes) and an H 2 blocker or antacids (to reduce acid-stimulated release of secretin, increasing the flow of pancreatic juice). Too often, these measures do not relieve pain, requiring increased amounts of narcotics, with the threat of addiction. Medical treatment of chronic pancreatic pain is often unsatisfactory. What Is China Likely To Do And Capable of Doing Next?

1 Introduction China stands on the edge of almost limitless possibilities and in order to attempt an answer to this question I need to limit the scope of the answer to fall within my fields of probable expert knowledge and my personal interests. These categories of my interest and possible Chinese advances are listed below. You should note that: I have no connection with the Chinese Government, although I admire the Chinese culture and its recurrent economic competence I have no connection with any pharmaceutical company My forecasts -of what the Chinese Government may do next - are based upon extrapolations of past policies and upon the new industries inherent in the higher latency of the 5G and 6G faster-internet-communication systems My forecast of the massive development of the megacity of Jing-Jin-Ji, formerly China’s Capital Economic Zone, is principally a reflection and extrapolation of of current Chinese Government economic policy. But the construction and creation of this intended 130-million population highly advanced Chinese Capital region will not only create the “biggest urbanized megalopolis region in Northern China ” but perhaps also the most important area of inventive and innovative development in the world, rivalling “the Pearl River Delta in the south and the Yangtze River Delta in the east.” This article is about China. Many of the medical protocols in Section 2 have a direct relevance to the people and nations of the West but their profit-focused health services with narrow medical education and profit-focused pharmacies do not seem to be interested in these issues, although there is vast Western research supporting these protocols which are not widely recognised or used. 2 Chinese Health Developments, particularly the increase in the health of Chinese people This paragraph of my Answer is an escalating-technology answer, starting from simple one-product protocols and ending with the application of Professor Andrew Sinclair’s complex epigenetic understandings. 2.1 The Reduction of Alzheimers’ among Chinese People Alzheimers’ appears to be the most prevalent of the central nervous systems diseases in China with an estimated 9.5 million diagnosed patients. This ‘treatment gap’ is certainly much greater in low and middle income countries, with one study in India suggesting 90% remain undiagnosed. If these statistics are extrapolated to other countries worldwide, it suggests that approximately three quarters of people with dementia have not received a diagnosis, and therefore do not have access to treatment, care and organised support that getting a formal diagnosis can provide. And modern science has discovered that poor dental hygiene is probably be the major cause of Alzheimer’s” so the size of the Dementia problem could be five to ten times larger than the numbers of people currently diagnosed. Once there, Pg can cause pathological changes. “Potempa and other researchers observed Pg in the brains of deceased people with Alzheimer’s disease. But what was truly surprising was “finding Pg major proteins, called gingipains, in the brains at a level much higher than in mentally healthy people of the same age,” Potempa said. “And when mice were orally infected with Pg, the same DNA fingerprint was discovered in their brains, as well as the changes typically seen in people with Alzheimer’s disease. “This result not only strongly supports that Pg brain infection may underline Alzheimer’s disease, but it pinpoints gingipains as major factors,” In other words, much of Alzheimer’s (as well as pneumonia, rheumatoid arthritis, heart disease, hepatitis, and oesophageal/throat cancer) could be prevented by better dental hygiene. But how best could China and the Chinese people cheaply and effectively do that? 2.1.1 The Prevention of Alzheimers By Small Daily 4g Doses of Xylitol in Morning Tea or Coffee Xylitol is a false sugar. See Xylitol - Wikipedia It does not kill bacteria directly but starves them by locking on to them and not letting them go. See 2.1.2 Preventing deaths from Pneumonia By Using Xylitol See Effect of Xylitol on Growth of Streptococcus pneumoniae in the Presence of Fructose and Sorbitol It looks as if pneumonia can be completely prevented by small safe doses of Xylitol in your morning tea. The Wikipedia entry on Xylitol used to say so. 2.1.3 Summary The health benefits of Xylitol can be summarised as And it provides an-almost-always-clear respiratory system, prevents pneumonia and Alzheimers’ and ear infections, and is the safest sugar substitute. A 1kg bag looks like this: Xylitol can be bought by the kilo. It costs about £10 per kilo in the UK, and there are 250 4 mg teaspoons in a kilo, so the UK cost of using Xylitol to clear the respiratory tract so you can continually enjoy a clear mouth, nose and throat system is about 4p a day. Not a high price to pay for an amazing range of benefits. And prevention is much better than cure, because it makes cure unnecessary. China is by far the largest world producer of Xylitol. But the Chinese people do not use Xylitol as frequently or as appropriately as they should. And of course if you eat sweets immediately after drinking lovely hot Xylitol-sweetened Chinese tea, then you destroy the mouth, brain and blood protection system you have just set up. Hundreds of millions of Chinese lives could be improved in quality and lengthened by the Xylitol protocol. I hope they will be. In my experience many British dentists know all about Xylitol. British doctors do not. Everyone should. 2.2 The Reduction and Possible Removal of Metabolic Syndrome Diseases The great Australian scientist Professor David Andrew Sinclair has made enormous advances in epigenetic understanding. If you take 1000 mg of resveratrol a day, then the sugar-caused Metabolic Syndrome diseases are likely to be massively reduced. This is no small matter. Taking the three pills he takes produces a new kind of human being, a person with a Superior Immunity Defence System (SIDS). (These daily pills are 500mg of Metformin, one gram of Resveratrol and 750 mg of NMN.) There seem to be seven major implications of this advance: The length of life becomes indeterminate (or more accurately, because 75% of the usual medical causes of death are removed, it becomes impossible accurately to calculate the average probable lifespan) The major effect which ends the lives of SIDS-enhanced individuals are likely to be accidents because as the Chinese sage Chan Chung told Genghis Khan the eternal truth that “Life can be extended, but death cannot be prevented.” And as Dr Roy Lee Walford, a previous longevity researcher, reported on pages 23–24 in his best selling book Maximum Life Span, W W Norton & Company, New York and London,1983: “If human aging were completely preventable but accidents continued at the same rate as today, the 50 percent survival can be calculated as around 350 years and maximum survival 600 years.” The Chinese Government could extend the life and lengthen the healthspan of the Chinese people by adopting Sinclair’s current protocols. These consist of a daily dose of “ 500mg of Metformin, one gram of Resveratrol and 750 mg of NMN.” I estimate that the average increase in lifespan might be no less than twenty years and no more than 50 years but the continued consumption of some Chinese high-sugar or high-fat diets could substantially reduce these. The Chinese Government is highly informed about these issues. As stated in An eMail to A Few Friends which contains the reference and the observations: “At present, the radiation on a trip to Mars would kill the occupants of the spaceship, but Sinclair’s research is very likely to provide a survival pill for that journey, with profound implications for human life on Earth.” and China as usual is at the forefront of this issues I remark at section 4.2: “China The most intelligent government in the world — the Chinese one — are in discussions with Prof Sinclair. The highest levels of the Chinese Government appear to be involved in these discussions, as Professor Sinclair reports at David Sinclair’s beginner’s guide to anti-ageing “In passing, he [David Andrew Sinclair] mentions he’s advising a large Asian nation (China, he clarifies after some questions) on how economic growth and a healthy population intersect. “This gets down to how do you transform the planet economically and not just [with] health,” he says. “The advice I’m giving that nation — and it’s at the highest level — they now want to know how do they raise up not just the economics, but the health [of their people]. Because they are very clever, they know [the two] have to go hand in hand.” As they do. By adopting the three-tablet Sinclair Protocol for all Chinese over 40 years old for all the 90 million in the governing class of China plus their wives. and all their people, China could completely bypass the dependency trap. The Sinclair protocol not only extends lifespan but it also increases the period of healthy life, so workers remain very productive throughout their working life. China could not only have the largest and culturally greatest economy in the world but also it could have the healthiest people. This new system of death control might increase the Chinese population by about 1.05% a year, rather than about half that, for at least twenty years. This will increase China’s population by about (1.0105)^6 or by 6% by 2025 and by (1.0105)^32 or 40% by 2050. Life expectancy will increase to over a century by 2025. The Chinese population is therefore likely to increase from 1,384m. to about 1,457m by 2025 and to about 1,938m by 2050 “ceteris paribus” (=if everything else stays equal) but things never stay the same. The Chinese removal of the dependency trap is a strong possibility. 2.4 Other issues 2.4.1 Why are cheap useful effective pharmaceuticals not prescribed or used much in the West? As I have said at Section 2 of What’s Wrong With Big Pharma? “The Business of Big Pharma is Making High Profits “The large pharmaceutical companies are in business to make a lot of money. They are not so interested in producing generics where the patent rights have expired or in other substances which may be biologically useful but which cannot be patented” and “There is a Big Pharma code phrase for useful but unpatentable products. They have “not been properly tested” because since big Pharma cannot make large amounts of money out of them, they have not been and never will be tested by them.” 2.4.2 A NaPCA Spray In the Kitchen Can Immediately Cure Small Burns Chinese mothers and fathers and the operators of commercial kitchens should place a couple of NaPCA sprays in their kitchen. The immediate spraying of NaPCA on the skin, after an accidental hot contact burn by water, oil or hot oven trays or hot parts of pots or ovens, immediately restores the skin’s natural humectant and the usual skin blisters will not develop. NaPCA can also be immediately used to restore sunburns, but the delay in use often makes it less effective. The NaPCA prevention of the development of skin burns in children and young adults seems almost miraculous. The damage done by skin burns can be substantial. This is the only way to prevent these. NaPCA is a preventative and not a restorative, and its delayed use is not as effective although it may help a little. In my humble opinion, the Chinese Government should invite Professor David Andrew Sinclair to establish a new academic chair/department either at the world class University of Shanghai Jiao Tong University or through the creation of a new medical research centre at Tsinghua University, at Beijing, or both. It seems inappropriate that China should not be one of the leading nations in the biomedical advances in life extension and I think that situation should be corrected by appropriate Chinese Government action and investment. The current small-scale trials of human life extension drugs in Boston, in New South Wales, and in Japan are interesting but the small scale is inappropriate and the results are slow to emerge.
