Is a V twin motor such as the Honda VT250 or 500 smoother and more "balanced" than a vertical twin such as a CB500?
The twin with the smoothest inherent design would be a 90 degree V twin such as built by Ducati or Moto Guzzi or a flat opposed twin such as BMW sells. Those wide V angle twins are relatively hard to package so most motorcycles narrow the vee to a smaller angle; the VT 500 was an example of how the narrow angle was ‘corrected’ by using a crankshaft with offset crank pins, effectively making it run as if it were a 90 degree twin. Most modern Vee twin engines have that offset crank pin or a balance shaft to squelch vibration. The old parallel twins many English bikes once had featured both pistons going up and down together, at the same time, but with the 2 cylinders on opposite strokes of the 4 stroke cycle-that enabled smooth, even firing but considerable mechanical imbalance. Modern parallel twins more often have one piston up when the other is down, but both have mechanical imbalance and so today’s parallel twins use various types of balance shafts to compensate for the inherent imbalance as do modern 3 cylinder engines. One might consider the offset crankpin to be a more ‘elegant’ engineering solution for the V twins, compared to adding a balance shaft and the required additional chain or gears needed to turn it. Many designs use the balance shaft to serve some additional function though. The big advantage of a parallel twin is they are cheaper to make and more compact so easier to package in the bike. Intakes and exhaust are in pairs so those parts are easier to mount also. The CB 500 has an especially good record of reliability,

When I was young, I got my motorcycle learners permit, and purchased a Honda VT250. I was riding out on the country roads, enjoying a ride, and riding within the speed limit. I get pulled over by a motorcycle policeman, so i pulled over to the side of the road, turned my bike off, put up the side stand, took off my helmet. He, did the same but left the lights flashing on his bike. I asked why he pulled me over as I was riding in the speed limit. He just wanted to look at my bike because he liked it. Just them two families from church drove by going to pick apples at the orchard right across from where I pulled over. They all pointed at me and laughed at me thinking i was speeding. How embarrassing!
